Plan to Return to Campus-Based Operations

Rensselaer has implemented its Plan to Return to Campus-Based Operations for the 2020-2021 academic year, in accordance with the provisions set forth by Governor Andrew Cuomo. All of us have worked hard to implement health and safety protocols to keep members of our community safe. With the arrival of our students, researchers, faculty, and staff on campus, many changes in the way our campus community must operate have been implemented, in order to continue to provide a safe living, learning, and working environment during this COVID-19 pandemic.

It is imperative that all members of our community are diligent in conforming to the established safety protocols. One of these protocols is utilizing the Institute’s “Health & Wellness Check-In” app. All faculty and staff who physically work on campus at any time must access the “Health & Wellness Check-In” app and complete the Daily Health Check and the Daily Interactions & Activity Log.

Daily Health Check

The Daily Health Check is used to identify potential COVID-19 symptoms. When you access the Daily Health Check, you will be provided a list of COVID-19 symptoms and you will check a box if you are currently experiencing any of the symptoms. In addition, you are required to indicate if you have tested positive for COVID-19 and have you been in close proximity with an individual who has been confirmed, or is suspected, to have COVID-19.

The Daily Health Check must be completed prior to the start of your normal work shift. Please note:  After your initial completion of the Daily Health Check, the button will no longer be active. If you develop symptoms after your initial completion of the Daily Health Check, you should access the “Daily Health Check-in” button on the left side of the page to report symptoms.  

Daily Interactions & Activities Log (DIAL)

The Daily Interactions & Activities Log was developed to assist you in recording your daily interactions with people, as well as the buildings, meeting rooms, etc. you have visited. Specifically, the log should capture names of individuals you have had close contact with (defined as six feet or less in proximity, for five minutes or more, and/or at any distance without masks). This requirement does not include individuals with whom you reside.

The Daily Interactions & Activities Log must be completed for each day of the week (Monday through Sunday). The information you provide will be used to assist in identifying individuals who may have been exposed to the virus that causes COVID-19.

The information you provide will be used solely for the health and well-being of you and other members of the Rensselaer community, and no other reason. Additionally, the information you provide will not be shared with anyone outside of Rensselaer, except as required by law, and will be retained no longer than 28 calendar days. The implementation of this app is consistent with the ethical principles of Public Health Contact Tracing procedures that balance the rights of individual privacy and confidentiality with public health and safety.
